We have an opportunity for two Behavioral Health Case Managers (CMs) in Charlotte and Raleigh North Carolina.

Key duties and responsibilities:

– Tracking medical status of assigned patients.
– Responsible for medical charting in electronic CM database.
– Request profile exams/evaluations and audiology exams at physical exam center.
– Coordinate patient treatment plans with appropriate medical facilities.

Education and Experience:

– Bachelor’s or Master’s degree in Social Worker or Psychology from an accredited program.
– Requires at least three years experience for Bachelor degree candidates or one year for Master’s degree candidates.

Work Hours:

– Minimum 40 hours per week
– Scheduled shifts are normally days Monday through Friday.
– Some weekend shifts required (1-2 times a quarter)
